Prevention and Population Health Program

Learn How You Can Improve Your Health

Trillium takes a population-based approach to improve the overall health of our members. We help promote targeted public health initiatives to address physical and mental health issues experienced in our communities. Our communities are only as strong as our people. Some examples of the issues we target include weight management, diabetes management, heart health, and childhood health.
