North Carolina ROOTS Survey
What is the NC ROOTS Project and how can you be involved?
Earlier this year, North Carolina received a $213 million grant to transform rural health services. The Rural Organizations Orchestrating Transformation for Sustainability (ROOTS) initiative provides a critical opportunity to improve the lives of those living in rural North Carolina.
Trillium Health Resources was selected to serve as the hub lead for our region, and we need your help. Your voice will directly help us identify:
- Community Needs: Where care is lacking most.
- Service Gaps: Programs or resources that are missing.
- Access Issues: Barriers preventing people in your county from getting
care.

About the NC ROOTS Advisory Board
The NC ROOTS Region 5 Advisory Board is being established to provide guidance for the NC ROOTS Hub Action Plan for Region 5. The Board consists of twenty-one (21) members and is designed to represent diverse communities, sectors, and lived experiences across the region. Advisory Board members play a vital role in supporting equitable, community-driven outcomes that strengthen the implementation, sustainability, and long term success of NC ROOTS initiatives.
Application Process Now Closed
The application period for the Region 5 NC ROOTS Advisory Board is now closed. We appreciate the strong interest from leaders across all sectors who are committed to strengthening rural communities, improving health equity, and expanding access to whole‑person care in Region 5.
