Annual Letters and ID Cards

Trillium Health Resources is required to send Tailored Plan members information about our privacy practices each year. Mailings will go out in May. You do not need to do anything other than review the information. 

In addition, we will start sending Tailored Plan members a copy of their Medicaid ID card every 12 months. These cards will start mailing in May as well.
