Help Shape Provider Learning at Trillium

To Trillium Contracted Providers and Staff

Trillium Health Resources invites contracted provider organizations and provider staff to complete the 2026–2027 Trillium Provider Learning Needs Assessment. Provider feedback will help inform Trillium’s learning priorities, training formats, scheduling and provider support for the 2026–2027 planning cycle. Survey dates: 

• September 14–October 2, 2026 
• Estimated completion time: Approximately 10–12 minutes 

The assessment is intended for Trillium-contracted provider organizations and provider staff across physical health, behavioral health, intellectual and developmental disabilities, Innovations Waiver, residential, outpatient and Tailored Care Management services.
