To support this transition and ensure that your organization can continue serving Trillium members without any disruption in payment, Trillium is partnering with Clearlink Partners to facilitate contracting with physical health and long-term services and supports (LTSS) providers. Clearlink Partners has started reaching out directly to providers help complete this process. They will assist Trillium in working with providers to add an amendment option for physical health providers that currently have a Tailored Plan Behavioral Health Contract with Trillium.

Contracting Changes for Trillium Physical Health Network
Trillium Health Resources is making an important update to our provider network management. Effective Oct. 1, 2026, we will directly manage our Physical Health and Long-Term Services and Support (LTSS) provider network through a partnership with Carolina Complete Health. This transition allows us to enhance our provider relationships, streamline billing and paperwork processes, and better support your practice. Trillium will be transitioning to a unified claims system for Tailored Plan operations as part of this process. Please visit this page about the new unified claims platform
Trillium invites providers to engage in a direct contract with Trillium, which would be effective Oct 1. If a provider does not contract with Trillium by Oct. 1, providers will continue to operate under the existing agreement with Carolina Complete Health, with Trillium acting as a third-party payer for claims. All Physical Health and Behavioral Health claims will be processed by Trillium for Tailored Plan operations, with the exception of NEMT and vision claims that will continue to be processed by Modivcare and Envolve Vision, respectively.
