Trillium Selected as NC ROOTS Hub


Trillium Health Resources Selected as NC ROOTS Hub Lead for Regions 2 and 5, Advancing Rural Health Across North Carolina
GREENVILLE, N.C. – Trillium Health Resources acknowledges its selection by the North Carolina Department of Health and Human Services (NCDHHS) to serve as the NC ROOTS (Rural Organizations Orchestrating Transformation for Sustainability) Hub Lead for Regions 2 and 5 as part of the state’s Rural Health Transformation Program (NCRHTP).
As part of this statewide initiative, Trillium, along with four other NC ROOTS Hub Leads will help support implementation of the NCRHTP across all 100 counties, advancing efforts to improve access to care for rural communities and strengthen coordination across health care systems.
Over the next five years, NC ROOTS Hubs will support regional and local partners in efforts to strengthen rural health infrastructure through initiatives focused on improving behavioral health and substance use services, enhancing care coordination across regions, leveraging digital tools to improve outcomes, expanding access to primary care, and strengthening the health care workforce.

Stevens Amendment Disclosure: $ 78,498,354 or 100% of the total costs of the North Carolina Rural Health Transformation Program in budget period 1 are funded with Federal funds, including awards anticipated under this RFA; $0 or 0% of the total cost of this program is funded by non-governmental sources. This disclosure is provided in accordance with the Stevens Amendment (Section 505, Division H of Consolidated Appropriations Act, 2021, Public Law 116-260).
