Trillium Regional Assistance Centers

The Trillium Regional Assistance Centers (TRAC} mark a major upcoming investment in regional infrastructure designed to consolidate innovation pilots, care coordination, and member support services into centralized hubs. With an initial $5 million investment, TRAC centers will serve as co-located service sites, designed to strengthen both provider network capacity and member access across Trillium's service regions. 

To guide planning and operations, Trillium conducted 2025 site visits to two national models: the FACT Center {Virginia Beach) and Morgan's MAC {Texas). Insights from these visits shaped the blueprint for a collaborative, service-driven model that emphasizes physical presence, cross-sector coordination, and access to wraparound supports. 

TRAC will serve as a physical and symbolic anchor for Trillium's long-term commitment to integrated care, regional innovation, and community-based service transformation.
