Access Point Booths
Trillium has several locations where you can complete the Access Point assessment in communities we serve. The displays meet current Americans with Disability Act (ADA) requirements if you use a wheelchair or walker. These booth offer you the opportunity to:
- Sit down to use the program to take a screening
- Take a wallet card with the Access Point web address for use at home
