Since July 1, 2024, over 8,000 members have safely arrived at more than 472,000 doctor appointments, thanks to Trillium’s coordinated transportation services. For many, these rides meant catching early signs of illness, staying on top of chronic conditions, or simply being able to get the care they need without barriers. Every ride is a step toward stronger mental and physical health—and toward brighter, healthier futures.

Through the Tailored Plan, technology is transforming independence for our members. Tools like the Trillium Ultimate Living Assistant (TULA), Attend Behavior, CHESS, and smart home communities like Francis’ Cove are empowering members to take control of their health. Whether it's setting medication reminders, communicating instantly with care teams, or receiving real-time remote support, these innovations are helping members live safely, confidently, and independently in their communities.

Today, nearly 100,000 members are receiving personalized support through Tailored Care Management—22,637 with Trillium’s own TCM teams and 75,474 through our partners. Behind each number is a story: a person managing diabetes more effectively, a parent finding the right pediatrician for their child, or an individual getting help navigating complex systems. Our Care Managers are true partners, guiding members toward healthier, more stable lives every day.
